Please read all information before submitting a request for a Hyperacusis consultation.
***Note: Consultation with Dr. Silverstein is to determine if Minimally Invasive Hyperacusis Surgery - Enhanced Round and Oval Window Reinforcement Procedure is an appropriate treatment option.***
Notice of Privacy Practices
Consultation Information and Scheduling:
- Complete Patient Forms
- Audiogram and Loudness Discomfort Level (LDL) files must be uploaded with patient forms. If you have a CT scan of the Temporal Bone (CT IAC), we kindly request that you upload it along with your patient forms.
- If you have any issues with file size or file type and are unable to upload your tests, please email info@EarRF.org for assistance or guidance.
- The Notice of Privacy Practices document is linked above for your review.
- After you submit your patient forms, a donation link will be listed in the confirmation pop-up on your screen.
- $375 donation to the Ear Research Foundation will be required at the time of scheduling.
- Rescheduling can be done no later than one week before the consultation.
- Cancelations or no-show appointments will not be refunded.
- A staff member will contact you to arrange a time and provide a link to log in for your consultation.
- ***Hyperacusis consults are currently being scheduled on Wednesdays between 4 PM EST and 5 PM EST***
- Additional times on Wednesdays, occasionally, are available for consultations.
- Email info@EarRF.org if Wednesday, 4 PM - 5 PM EST, does not accommodate your schedule.
- Please log in at least 15 minutes before the scheduled time.
- After consultation, if Dr. Silverstein refers you for Hyperacusis surgery, we will email you a Medical Authorization Form for completion, which will enable us to send your records to the surgeon.
