The Ear Research Foundation is providing Communication Cards for FREE to anyone that submits the request form online, in-person, email, or mail.
The Foundation will mail your Communication Card to address provided on form.
Communication Cards are to assist individuals with hearing loss or a hearing impairment. The card can be kept on you, in your bag or briefcase or in your vehicle. If you ever find yourself in a situation unable to effectively communicate with someone, show them your Communication Card, to best assist with effective communications.
